Dossier of application for Certificate of food safety conditions
This is the content stated in Circular 43/2018/TT-BCT on food safety management (FHS) under the responsibility of the Ministry of Industry and Trade.
Accordingly, for cases of first-time application for certificates of food safety conditions, the dossier includes:

- Application form Form No. 01a.

- Explanation of material facilities, equipment and tools to ensure food hygiene and safety according to Form No. 02a (with production establishments), 02b (with business establishments) or Form No. 02a and 02b (with establishments) both manufacturing and trading.

- Certificate of full health / List of confirmed health status of the owner and the person directly producing and trading in food issued by the medical establishment at district level or higher (certified copy of the certificate) Department).

- Certificate of training in food safety knowledge / Certificate of food safety knowledge of the owner and the person directly producing and trading food (certified copy of the establishment).

Thus, the first-time application is no longer required to submit business registration certificate / business registration certificate / investment certificate with food production and business lines compared to the current regulations in Circular 58/2014/TT-BCT dated December 22, 2014.

Circular 43/2018/TT-BCT takes effect from 01/01/2019.
