Application for granting Food Safety Certificate
Annex VI Circular No. 45/2014/TT-BNNPTNT

The Certificate of Business registration or the Investment certificate containing the food production/trading branch or the certificate of farming economy (except for the mechanic fishing ship with total capacity of the main machine of 90CV or higher):
Notarized copies or the copies enclosed with the original for comparison

A description of the food safety conditions of the establishment according to Appendix VII enclosed with Circular No. 45/2014/TT-BNNPTNT
(except for the mechanic fishing ship with total capacity of the main machine of 90CV or higher)

The list of owners of establishments and people directly carry out the production/trading of food who are issued with the Certificate of proficient in food safety to the same recipient 
(verified by the production/trading establishments);
5
The list of owners of establishments and people directly carry out the production/trading of food who are issued with the Certificate of satisfactory health
(verified by the production/trading establishments).

Written registration for inspection of veterinary hygiene standards for aquatic products

Report of remedies or elimination of violations in veterinary hygiene requirements specified in the inspection record
 
 
 
Process Steps
Step 1
 
Submit the dossier of registration for inspection of veterinary hygiene standards to the competent agency of veterinary hygiene
Step 2
The Department of Animal Health shall examine the completion and validity of the dossier. If the dossier is incomplete and invalid, it shall guide the establishment owner in completing the dossier according to regulations
Step 3
Establish the Inspection Team
Step 4
The Inspection Team shall inspect veterinary hygiene conditions and take sampling (if necessary) at the establishment.
Step 5
The laboratory shall issue the result notice
Step 6
If the establishment fails to meet veterinary hygiene requirements, the establishment owner shall remedy or repair unsatisfactory items and request for re-inspection.
Grant the Certificate of veterinary hygiene standards for aquatic products if all conditions are satisfied.
